ENROLLMENT FORMS The following is a list of forms that you will need to complete for each child registered at Casas Christian School. The files are in PDF format (Adobe Acrobat Portable Document Format) and requires the Adobe Acrobat Reader. To download your free copy of the Acrobat Reader, click on the icon below:DIRECTIONS: Each form will open in a separate window. Open the file, print the form, and close the PDF file (or website) when you're finished. You will return to this window. The following forms must be completed as part of the Enrollment Packet: Application, Emergency Information and Immunization Record Card, First Aid Product Release, After School Activity Form, and the Financial Commitment Form. You will also need to provide copies of your child's immunization records, birth certificate, report cards for past two years, and standardized testing for past two years. The Volunteer Survey and Mentoring Program Form are optional. Please use the Enrollment Application Checklist to make sure you have included all necessary items in your Enrollment Packet.
