FORMS

The following is a list of forms that you will need to complete for each child registered at Casas Christian School.  The files are in PDF format (Adobe Acrobat Portable Document Format) and requires the Adobe Acrobat Reader.  To download your free copy of the Acrobat Reader, click on the icon below:

Directions:  Each form will open in a separate window.  Open the file, print the form, and close the PDF file (or website) when you're finished.  You will return to this window.

The following forms must be completed as part of the Enrollment Packet:  Application Form, Emergency Information and Immunization Record Card, First Aid Product Release Form, Student Health History Form, and After School Activity Form.  You will also need to provide a copy of your child's immunization records and birth certificate.  The Volunteer Survey and Mentoring Program Form are optional.  Please use the Enrollment Application Checklist to make sure you have included all necessary items in your Enrollment Packet.  Attach the Checklist to your enrollment forms and turn into the School Office.
